A plane appeared to bounce after landing at the Caribbean Island airport of Sint Maarten and skidding down the runway.
The report states the pilot did not slow the plane enough when he turned from the runway to the taxiway. The runway was wet, and when the left landing gear left the paved surface, it hit a manhole that served as an electrical junction box.
Chaotic video from one of the passengers shows a chaotic scene as people went down the slide.
"While they are all unrelated, I want you to know that these incidents have our attention and have sharpened our focus," United CEO Scott Kirby said.
An IAH spokesperson said everyone was evacuated safely and no injuries were reported. The FAA said United Airlines Flight 2477 from Memphis rolled onto the grass while exiting onto the taxiway.
A Southwest Airlines passenger remains hospitalized after opening an emergency door and jumping out of the plane while it was still on the tarmac in New Orleans.
