Tesla driver manually overrode FSD, reached speeds of more than 70 mph in deadly crash: NTSB report

HARRIS COUNTY, Texas (KTRK) -- A report into a Tesla crash that left a 76-year-old woman dead in the Katy area last month has just been released.

On Wednesday, the National Transportation Safety Board (NTSB) released a preliminary report regarding the June 19 incident.

Michael Butler, identified as the Tesla driver, is charged with manslaughter after he allegedly crashed into a home on Rose Hollow Lane, killing 76-year-old Martha Avila.

Video provided by the family shows the vehicle plowing directly into the home. Avila suffered catastrophic injuries and later died.

According to NTSB'S report, "the driver manually overrode FSD (Supervised) by pressing the accelerator pedal to 100%, and the vehicle's speed was greater than 70 mph when the crash occurred."



Investigators said Butler claimed his car was in self-driving mode, but Tesla disputed that and claimed he overrode the feature.

Butler was booked into jail at the beginning of this month. It's unclear if he has bonded out.
